I declare that I have read the internal reporting procedure.

I declare that I consent to the processing of my personal data for the purpose of accepting the report and taking follow-up actions based on the internal reporting procedure.

I acknowledge and agree that the administrator of my personal data is Enbio Technology Sp. z o.o., with its registered office in Rumia at Działkowców 15.

I acknowledge that false information reporting or reporting by the perpetrator of the violation of the law, which is being reported, is not protected under the Whistleblower Protection. Moreover, the person who has reported false information regarding the violation of the law is subject to criminal liability and may be fined, sentenced to restriction of liberty, or imprisonment for up to 2 years (Article 57 of the Whistleblower Protection Act).

I acknowledge and agree that in the event of proceedings being initiated by competent state authorities based on the information provided in the report, my personal data will be transferred to the competent authorities, which may require me to participate in such proceedings as a witness, party, etc. (depending on the content of the report and the role of the submitter in the incident).

Ich bestätige, dass jeder, der bei einer Zeugenaussage, die als Beweismittel in einem Gerichtsverfahren oder einem anderen nach dem Gesetz durchgeführten Verfahren dienen soll, die Unwahrheit sagt oder die Wahrheit verschweigt, mit einer Freiheitsstrafe von 6 Monaten bis 8 Jahren bestraft wird (Artikel 233 § 1 des Strafgesetzbuches).
